What are the responsibilities and job description for the Hospice Licensed Vocational Nurse (LVN) - Per Diem position at NEXT CARE HOSPICE?
We are seeking a full-time LVN Licensed Vocational Nurse to join our team! You will be responsible for delivering high-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable.
Responsibilities:
- Provide Skilled Nursing Care in a home or facility setting
- Ensure the appropriate use of meds, DME, and supplies
- Educate and support patients, families, and caregivers
- Participate in IDT and updating the Plan of Care
- Provide our patients and families the best care possible
- Daily travel is required when working
Qualifications:
- Valid LVN License in California
- One year or more of experience in either Home Health or Hospice is preferred, but not required
- Previous experience in a hospice setting is a plus
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
Salary : $45 - $60
