Next Care Hospice is a compassionate and dedicated hospice agency committed to providing high-quality end-of-life care. We strive to make a meaningful impact on the lives of our patients and their families.
We are seeking an experienced and dynamic Hospice Agency Liaison to join our team.  The Hospice Liaison serves as the connecting link between patients, families, and hospice care services, ensuring a smooth transition and personalized care plan tailored to each patient’s unique needs. This role involves educating and consulting with healthcare providers, patients, and families about the benefits and services provided by hospice care, facilitating a deeper understanding and acceptance of this compassionate approach to end-of-life care. By working closely with medical professionals and care teams, the Hospice Liaison helps to coordinate care that honors the patient’s wishes, promotes comfort, and supports the emotional well-being of both patients and their loved ones during a sensitive time. Through their efforts, they play an instrumental role in making hospice care accessible and understood, ensuring that patients receive the dignity, respect, and quality of life they deserve.
