WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …
We are seeking an experienced Case Manager to join our Healthy Transitions team. The Case Manager is responsible for actively engaging and collaborating with clients to reduce barriers to achieving positive outcomes in critical areas of life.
Main Duties and Responsibilities:
- Participate in team meetings to review client visits, concerns, and coordinate care.
- Communicate with client families and support members as part of the client’s recovery plan.
- Participate in supervision with licensed staff and contribute to the development of treatment plans.
- Document services within the electronic health record.
- Consistently provide a safe and therapeutic environment.
- Adapt care “style” to meet needs of various children/adolescents.
WHAT WE EXPECT OF YOU…
- Associate’s degree required; bachelor’s degree preferred. Degree in Human Services field preferred.
- 2 years of experience in the Human Services field preferred.
- Valid Driver’s License with reliable transportation to travel to facilities across the area.
- Exceptional verbal and written communication skills, with the ability to engage effectively with diverse audiences.
- Strong organizational skills with the ability to prioritize work and meet deadlines.
- Self-motivated, independent, and adaptable in dynamic work environments.
- Collaborative mindset with the ability to work well within multidisciplinary teams.
- A willingness to tackle complex tasks in an independent manner
- Problem-solving skills with the ability to apply critical thinking in client care situations.
