Job Description

A staff pharmacist is responsible for providing quality pharmaceutical care for the patients of The Stamford Hospital with an emphasis on accuracy and patient safety.

MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES:

Accurately reviews, verifies, prepares, and dispenses medication orders per physician request according to established policies/procedures/protocols and based on pharmaceutical knowledge
2. Provides timely and accurate drug information to healthcare professionals and lay persons when required
3. Participates in unit based decentralized role providing medication counseling sessions for patients at time of discharge or when requested by reviewing side effects, indications, and directions for use
4. Demonstrates knowledge and skills necessary to provide quality pharmaceutical care to all patients
5. Maintains competency in clinical skill set and abilities
6. Ensures appropriateness of therapy and dosage for all patient populations including age-specific care
7. Monitors patient therapy in accordance with departmental policies/procedures, protocols, and programs.
8. Carefully identifies, prevents, intervenes, and follows up on any medication related problems, including any actual or potential adverse drug events
9. Responsible for day-to-day management of drug distribution, including all controlled substances, compounded sterile and non-sterile medications, hazardous medications, repackaged medications, and investigational drugs
10. Supervises pharmacy support personnel and coordinates operational workflow to ensure the seamless and timely delivery of pharmacy services throughout the institution
11. Completes required continuous training and education requirements, including state and federal regulated, hospital and departmental specific requirements.
12. Ensures safe and appropriate cost-effective medication therapies for patients according to established policies, procedures, guidelines, and protocols.
13. Assists with pharmacokinetic consult services and drug dosing per organizational protocol
14. Ensures that therapeutic substitution policies are followed based on individual patient need.
15. Participates in quality improvement and medication use review activities of the department
16. Participates in hospital initiatives and committees as assigned
17. Adheres to the hospital’s 340B requirements regarding ordering, dispensing, and transfer of medications within the hospital departments and to outside hospitals
18. Demonstrates fiscal responsibility
19. Abides by regulatory requirements, including but not limited to USP797, USP800, USP795, NIOSH, DCP, DPH, and Joint Commission standards
20. Maintains current knowledge of drug use through continuing education
21. Precepts pharmacy students as assigned
22. Provides drug information in-services as assigned
23. Participates in the development of quality initiatives as assigned
24. Demonstrates flexibility in working rotating shifts as assigned including days, evenings, overnights, weekends, and holidays.
25. Communicates effectively with nursing, physicians, and other healthcare professionals to provide optimal pharmacy services and to uphold the values of the pharmacy department by creating a positive relationship
26. Maintains proficiency with automation and technologies such as Automated Dispensing Cabinets, Carousels, Prepackaging machinery, Electronic Health Record, IV workflow solution etc.as needed to perform assigned responsibilities.
27. Prepares hazardous and non-hazardous sterile and non-sterile preparations and directly supervises technician compounding personnel.
28. Provides cross coverage, when requested, at Tully Surgical Center, Bennett Cancer Center, and Medication Therapy Management/Anticoagulation Clinic.
29. Participates in Medication Safety and Patient Safety incident reporting and safety initiatives
30. Adheres to all Hospital and Pharmacy Policy and Procedures as applicable to pharmacy operations and medication use.

COMPETENCIES AND WORK EXPERIENCE REQUIREMENTS:
Inpatient Hospital Pharmacy experience, preferred.

EDUCATION REQUIREMENTS (INCLUDE LICENSE, REGISTRATION, CERTIFICATION):

B.S. Pharmacy, Pharm.D. preferred
PGY1 Residency, preferred
License or eligible for license as a Registered Pharmacist in Connecticut
