Required Education and Experience
Must possess an Associate or Bachelor degree in Nursing and a valid Registered Nurse license issued by the Kentucky Board of Nursing or a state Nursing Board in the “Compact State Agreement”.
Position Type/Expected Hours of Work
Typical hours for this position are M-F 8AM - 4:30p. This is a FULL TIME 37.5 hour work week position. Employee must be able to flex hours/work overtime as needed to meet facility needs.
Essential Functions
- Attends all client related meetings on their assigned floor.
- Serves as the liaison between the ARNP and outside facilities.
- Attends medical appointments with clients as needed.
- Make rounds on assigned units and to hospitals as needed when clients are hospitalized; providing written update to the team.
- Complete all monthly physical assessments on clients.
- Completes all nursing assessments (to include but not limited to: Braden, Dehydration, Diabetes, Moses, TD)
- Attend specialty clinics as needed or requested
- Ensure immunization records are up-to-date
- Responsible for writing, monitoring and updating SAMS learning objectives and recording into Key Indicators
- Write mortality reviews
- Completes pre-admission assessments for potential clients, all needed paperwork for new admissions as needed.
- Assist with transition training related to client care
- Serves on facility committees, attends in-service training programs, participates in drills, and responds to facility emergencies.
Supervisory Responsibility
This position requires supervision of LPNs and RNs
Travel
Travel may be expected to cover nursing staff at the Group Homes location.
