Job Summary:
As a Licensed Practical Nurse (LPN), you will play a vital role in our care team, working closely with patients, families, and other healthcare professionals to provide high-quality care. We are seeking a dedicated and compassionate LPN to join our team on a part-time and full-time basis.
Responsibilities:
Administer medications and treatments as ordered by physicians
Monitor and record patients' vital signs and medical conditions
Develop and implement individualized care plans with input from the healthcare team
Perform routine nursing tasks, such as wound care and vital sign checks
Communicate effectively with patients, families, and healthcare team members
Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
Current LPN licensure in the State of New Jersey
Associate's degree in Nursing or related field
Minimum 1 year of experience as an LPN in LTC
Ability to lift and transfer patients with assistance as needed
Strong communication and interpersonal skills
