Role Description
This is a full-time, on-site position for a Supportive Services for Veteran Families (SSVF) Case Manager, located in Baltimore, MD. The SSVF Case Manager will provide case management services to veterans and their families, including intake assessments, service planning, and connection to resources. This role includes monitoring client progress, maintaining accurate records, and collaborating with community partners to ensure clients have access to necessary housing, employment, and supportive services. The Case Manager will facilitate crisis intervention, advocate for clients, and promote self-sufficiency among veteran families.
Qualifications
- Experience in case management, social work, or delivering direct supportive services
- Excellent interpersonal, communication, and advocacy skills
- Ability to coordinate and collaborate with community organizations and resources
- Strong organizational, documentation, and time management skills
- Knowledge of veteran-related resources, housing programs, and benefits is highly preferred
- Familiarity with trauma-informed care principles and crisis intervention techniques
- Proficiency in basic computer skills and database management
- Bachelor’s degree in Social Work, Human Services, or a related field, or equivalent experience
- Demonstrated compassion, cultural competency, and the ability to work with diverse populations
- Valid driver’s license and reliable transportation required
