What are the responsibilities and job description for the Case Manager position at New Roads Behavioral Health?

-Case load will consist of up to 33 clients (often less)
-Case managers will assess new admits, interact with families
-Meet with each client at least once a week and assist clients with needs that could include; skills training, program work, aftercare planning, legal issues, amongst many other necessities.
-Often connecting clients with outside resources such as disability, state services, etc.
-Diligent note taking and documentation of sessions, communication with families and intercompany communication.
-Attending staff meetings with therapists and providing/receiving updates on each client
-Experience with case management preferred but not required
-Working with medical team when necessary
-On occasion transporting clients to appointments
-Attend required trainings when needed
-HS diploma/equivalent -Bachelor's degree preferred not required
-Valid driver's license
-Can pass a background check
