Purpose (General Description and Summary) 

The Clinical Case Manager – Float provides community- and clinic-based interventions designed to support individuals with mental health diagnoses. This role is part of New Narrative’s clinical teams and will work across a variety of service areas, including outpatient, Intensive Case Management (ICM), and youth services, based on program need. The Float Case Manager implements, monitors, and documents services for participants while maintaining flexibility to adapt to different teams and settings. Case Managers are expected to provide outreach in the community and participants’ homes, deliver skills training, connect individuals to resources and peer supports, and demonstrate strong advocacy skills. 

Accountabilities (Responsibilities of the Job) 

The Clinical Case Manager – Float works in collaboration with QMHP Mental Health Clinicians to create and implement individualized service plans. As a member of multiple clinical teams, this position provides interdisciplinary care and wraparound services to support participants in accessing resources, meeting basic needs, accomplishing service plan goals, and attaining the highest level of independence possible. 

Case Management 

  • Assist participants in connecting to resources needed to achieve goals in their individual service plans. 

  • Advocate for participants across various systems of care. 

  • Learn and navigate Electronic Health Record systems; concurrently document participant interactions. 

  • Coordinate with Mental Health Clinicians to provide mental status assessments as needed. 

  • Develop relationships with providers and stakeholders to facilitate service provision. 

  • Maintain regular, frequent contact with participants. 

  • Coordinate with external providers and natural supports, including money management services, exceptional needs coordinators, prescribers, landlords, family/friends, and other community providers. 

  • Provide transportation as needed for participant appointments. 

  • Support individuals in obtaining vital documents. 

  • Connect participants to housing and culturally specific resources as applicable. 

Skills Training 

  • Collaborate with Mental Health Clinicians to incorporate appropriate interventions into service plans. 

  • Provide skills training related to ADLs and IADLs based on service plan goals. 

  • Utilize motivational interviewing and other therapeutic modalities to support recovery and symptom management. 

  • Lead therapeutic groups on topics such as recovery skills, symptom management, coping skills, communication, mindfulness, and physical activity. 

Crisis Prevention and Intervention 

  • Respond to crisis situations using sound clinical judgment and critical thinking. 

  • Employ trauma-informed practices and crisis prevention strategies. 

  • Use risk assessment tools to evaluate potential harm to self or others. 

  • Apply de-escalation practices and connect participants to emergency services as needed. 

  • Complete incident reports within 24 hours of crisis events. 

  • Coordinate respite referrals and oversee participant stays. 

  • Support hospital contact and discharge planning. 

Administration 

  • Document all client contact within 72 hours of service delivery in Electronic Health Record (Credible). 

  • Document all meaningful client interactions according to policy and Oregon Administrative Rules. 

  • Utilize and regularly review outcomes-based care tools. 

  • Participate in supervision bi-weekly to review interventions, documentation, and professional development goals. 

  • Attend and positively contribute to required meetings, including weekly staff meetings and interdisciplinary care coordination meetings. 

Experience/Qualifications 

  • Prior experience providing QMHA-level individual case management services and skills training groups. 

  • Minimum of two years of experience in social work or mental health preferred. 

  • Experience working with the SPMI population preferred. 

  • Ability to work independently and collaboratively within different teams. 

  • Knowledge and application of trauma-informed principles. 

  • Familiarity with Motivational Interviewing. 

  • Willingness to learn mental status assessments, psychiatric medications, administrative rules, and applicable laws/regulations. 

Other General Expectations 

  • Adhere to confidentiality, professional ethics, and dual relationship standards. 

  • Implement evidence-based practices and therapeutic modalities in participant care. 

  • Pursue ongoing professional skill development. 

  • Follow all organizational policies and protocols. 

  • Maintain a valid Oregon Driver’s License and ability to transport participants. 

  • Maintain CPR and First Aid certification. 

  • Maintain QMHA certification. 

Education/Certificates/Licenses/Registration 

Qualified Mental Health Associate (QMHA) certification with a Bachelor’s Degree in social work or related field required.
