We are seeking a full time (40 hours) compassionate and detail oriented Social Worker to join the team at New London Rehab and Care Center of Waterford. The Social Worker will support residents and their families by addressing psychosocial needs facilitating care planning and ensuring a smooth transition throughout the continuum of care. Bachelors or Masters in Social Work required.
