New Level Home Care is seeking an experienced, compassionate, and dependable Registered Nurse (RN) with home health experience to provide skilled nursing care to patients in their homes. The ideal candidate is organized, clinically competent, and committed to delivering patient-centered care while maintaining compliance with Medicare, Medicaid, and Virginia regulations.
Responsibilities
- Perform comprehensive nursing assessments in patients' homes.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and other required nursing assessments.
- Accurately complete OASIS assessments and documentation within required timeframes.
- Administer medications, injections, wound care, IV therapy, catheter care, and other skilled nursing treatments as ordered.
- Monitor patients for changes in condition and promptly report significant findings to the physician.
- Educate patients and caregivers on disease management, medications, safety, and preventive care.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Ensure all documentation is accurate, timely, and compliant with Medicare, Medicaid, and agency standards.
- Participate in case conferences and interdisciplinary team meetings.
- Maintain patient confidentiality and comply with HIPAA regulations.
- Respond promptly to patient needs and provide exceptional customer service.
- Perform additional duties as assigned to support quality patient care.
Pay: $50.00 - $70.00 per hour
