An employee in this position is responsible for the overall coordination of services in the residential program ensuring the safe, quality care of people by overseeing direct support staff. This includes adhering to OPWDD regulations, and services, as identified in the Life Plan and specified in the Staff Action Plan, that enable each person to live in the community. These activities and supports are uniquely tailored for and by the person rather than for the benefit of the group. The Residence Coordinator is responsible for the supervision and operation of the Individualized Residential Alternative (IRA). The schedule for this position is: Sun 8a-4p, Mon 1p-9pm, Tues 8a-4p, Wed 8a-4p, and Thurs 6a-3p. This person in this position needs to have some flexibility in ability to do hours.

Schedule: Saturday off, Sunday 8-4, Monday 1-9p ,Tuesday 8-4, Wednesday 8-4, Thursday6-2, Friday off, Flexibility based on program needs

Essential Skills and Experience:

  • Ability to physically assist individuals as required; and, to intervene physically when appropriate using approved techniques to safeguard the individual, oneself and others during behavioral crises.
  • Ability to assist in the evacuation of the community residence/van/facility in the event of an emergency (including but not limited to fires and floods) in a manner consistent with agency and with OPWDD regulations/guidelines.
  • Ability to work independently (when appropriate).
  • Ability to use sound judgment while following agency policies, practices and procedures.
  • Ability to speak, read, and write English which allows full communication and participation with individuals served, their families, agency staff, and government visitors.
  • Successfully complete all agency-required training within 90 days of employment and to keep required training status current on an ongoing basis.
  • Possess a valid driver license with a driving record acceptable to NHR’s insurance carrier and possess the ability to drive agency vehicles to transport individuals served to and from: medical appointments; day programs; and, other activities or events in the community at large.

Education, Training and Experience:

High School Graduate with one-year experience with adults with I/DD and three-years supervisory experience.

OR Associate’s Degree with one-year experience with adults with I/DD and two-year supervisory experience.

OR Bachelor's Degree with one-year experience with adults with I/DD and one-year supervisory experience.


Nonessential Skills and Experience:

  • Prior work experience handling multiple responsibilities and duties.
  • Prior work experience planning and implementing recreational activities.
  • Knowledge of community resources.
  • Computer proficiency, particularly with Microsoft Office applications.

Physical Demands and Work Environment:

The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions of this position.

While performing the duties associated with this position, an employee (using approved agency lifting techniques) will be called upon to support and lift adult individuals with developmental and other disabilities; transfer these individuals to and from wheelchairs; to and from beds; on and off toilet seats; in and out of showers; and, in and out of vans or other vehicles.

An employee in this position must be able to physically intervene (when appropriate and in an approved manner) to protect the individual from causing harm to themselves or others and to safely evacuate the community residence/van/facility during an emergency. These duties may entail lifting/supporting/transferring up to 50-70 pounds on one or more occasions during the work shift and occasionally up to 200 pounds with assistance from other staff or devices. Employees will regularly be required to stand; walk; sit; use hands (or hands and arms) to carry, finger, handle or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop; crouch and kneel. Employees will also be regularly required to hear (with or without hearing devices), see (with or without corrective lenses) and speak.

While performing the duties associated with this position, an employee will be in a primarily residential environment (not a typical office setting); in a van or other vehicle; or, in the community at large.

This position will at times require an employee to provide emergency coverage.

This position may expose employees to blood borne pathogens.

This position may expose employees to the profanity and offensive/violent behavior of individuals served.

This position may require staff to assist individuals served with very personal intimate care.


Training:

Successfully complete required training programs, including but not limited to SFA, CPR, Medication Administration, SCIP-R. Attend and participate in administrative meetings as required.

In a manner consistent with NHR’s Mission Statement and Unifying Principals and applicable laws and OPWDD regulations, an employee in this position shall:

  • Ensure progress toward achieving the values of NHR Mission Statement, i.e. achieve fulfilling lives within their local communities by establishing a home, belonging, self-determination, learning, productivity and individual respect.
  • Ensure home is maintained consistent with HCBS guidelines i.e. services that people can use to help them be healthy and reach their personal goals.
  • Ensure health, safety, and habilitation of the people we support is consistent with agency philosophy and department practices while maintaining a positive living environment.
  • Ensure the appropriate, therapeutically documented level of supervision in the residence, is adequate to protect the health and/or safety of the people we support.
  • Overseeing all financial systems, ensuring timely completion of household expenditures, including monthly cash on hand reconciliation's, weekly grocery disbursements, purchase orders, clothing money, etc. and maintaining records for the people we support and program expenditures.
  • Ensure the completion and maintenance of all paperwork and written records related to individual treatment. This includes but is not limited to medical records, treatment records, progress reports, data collection, behavior reports, incident reports, billing records, and all written communication systems.
  • Maintaining a safe environment for staff and the people we support and reporting any hazardous conditions.
  • Manage sensitive or crisis situations in a manner that is respectful and maintains the dignity of each person.
  • Developing professional, empathic relationships with family members ensuring they are included in the team.
  • Ensure participation in Life Plan meetings; identify special needs and coordinate with Care Managers for other meetings.
  • Leading and facilitating interdisciplinary team meetings and staff meetings to ensure effective communication.
  • Monitoring personal health and well-being, administering medications as needed, and coordinating with medical providers when necessary.
  • Ensure the delivery of routine and as needed medical and dental services in consultation with the RN and person involved.
  • Ensure all physician orders, including those for medication, are followed and in compliance with state regulations (Part 633).
  • Ensure that agency vehicles assigned to the program are maintained in a safe condition by conducting monthly inspections and documenting those inspections in Fleetio and by promptly reporting problems with vehicles through Fleetio.
  • Ensure general nutritional guidelines are followed in menu planning, and reflect the preference of people in the home.
  • Ensure compliance under the OPWDD guidelines through training, coaching, supervising and evaluating DSPs on the NYS Core Competencies and NADSP Code of Ethics.
  • Work varied schedule, including evenings and one weekend day required, to provide adequate contact with residential program, staff, family and advocates.
  • Represent the agency and program in a positive manner to staff, other agencies, the community and other service providers
  • Review and approve time sheets for accuracy and approve time off requests accordingly.
  • Develop and monitor staff schedules to ensure adequate coverage and minimize overtime and forward to DPS.
  • Review and investigate internal incident reports in a timely manner - ensure they are classified correctly and in compliance with state regulations (Part 624) related to incident reporting. When necessary ensure proper notifications are made to the NYS Justice Center, the Director of Program Services and the NHR quality department of any incidents that meet the definitions of reportable incidents or serious notable occurrences.
  • Ensuring a work environment that is free from harassment and discrimination.
