Job Description Summary

The Certified Home Health Aide (CHHA) is a vital member of the hospice interdisciplinary team, providing personal care and supportive services to patients in their place of residence under the supervision of a Registered Nurse (RN) or other licensed clinician. The CHHA assists patients with activities of daily living (ADLs), promotes comfort and dignity at end of life, and supports families during the hospice journey. This role requires compassion, reliability, strong communication skills, and adherence to state regulations, hospice philosophy, and agency policies while working in emotionally sensitive and diverse care environments.

Essential Job Functions / Responsibilities

Responsibilities of the Certified Home Health Aide include, but are not limited to, the following:

  • Provide personal care services such as bathing, grooming, oral care, toileting, and dressing in accordance with the hospice care plan.
  • Assist patients with mobility, transfers, positioning, and ambulation to promote comfort and safety.
  • Perform basic patient care tasks including skin care, linen changes, and light meal assistance as permitted.
  • Observe and report changes in patient condition, comfort level, or safety concerns to the supervising RN.
  • Provide emotional support and compassionate presence to patients and their families.
  • Reinforce comfort measures and hospice philosophy during patient interactions.
  • Maintain a safe, clean, and supportive patient environment.
  • Accurately document care provided, observations, and patient responses in accordance with agency policy.
  • Communicate effectively with patients, families, and interdisciplinary team members.
  • Follow infection control, safety standards, and patient confidentiality requirements.
  • Participate in hospice team meetings, in-service education, and required training.
  • Demonstrate professionalism, respect, and cultural sensitivity at all times.

Position Qualifications

  • High school diploma or equivalent.
  • Graduate of a state-approved Certified Home Health Aide (CHHA) training program.
  • Currently certified as a CHHA in the State of California.
  • Completion of required Home Health Aide hours and competency evaluations.
  • Prior experience in hospice, home health, long-term care, or caregiving preferred.
  • Demonstrates understanding of hospice philosophy and end-of-life care principles.
  • Effective verbal and written communication skills.
  • Ability to follow written care plans and instructions from licensed clinicians.
  • Must be a licensed driver with reliable transportation and automobile insurance in accordance with state and agency requirements.
  • Possesses and maintains current CPR certification.

Job Limitations

Certified Home Health Aides must practice within the scope defined by California regulations and hospice agency policies. CHHAs may not perform skilled nursing tasks, administer medications, assess patients independently, or alter care plans. All care is provided under the supervision of a Registered Nurse. Decision-making authority is limited to reporting observations and following established instructions. The role involves emotional challenges associated with caring for terminally ill patients and their families. CHHAs must work within resource limitations of home and facility environments while strictly adhering to infection control standards, safety protocols, confidentiality requirements, and documentation guidelines.

  • Emotional resilience and ability to cope with loss and grief.
  • Commitment to ongoing training and professional development.

Working Conditions

  • Providing care in patients’ homes, boarding care facilities, assisted living facilities, and skilled nursing facilities.
  • Frequent travel within the assigned service area.
  • Working independently while maintaining communication with the hospice team.
  • Exposure to patients with advanced illness and varying levels of physical dependency.
  • Physical demands including lifting, repositioning, bending, and assisting with mobility.
  • Emotional demands related to end-of-life care and family support.
  • Adherence to infection control and safety protocols.
  • Flexible scheduling, which may include evenings, weekends, and holidays.
  • Navigating varying levels of support and resources in different care environments.

Job Type: Contract

Pay: $35.00 - $40.00 per hour

License/Certification:
