Job Summary
Salaried position. We are seeking a dedicated and compassionate Resident Care Coordinator to oversee and coordinate comprehensive care services for residents in a newly opened 12 bed secure memory care unit. The ideal candidate will possess strong leadership skills, extensive experience in memory care, and a thorough understanding of care plans, medication administration, and operations management. This role involves working closely with residents, families, and healthcare teams to ensure personalized, high-quality care that promotes dignity and independence.
Responsibilities
- Develop, implement, and monitor individualized care plans tailored to residents’ with dementia or Alzheimer's.
- Coordinate medical and personal services ensuring compliance with healthcare regulations and facility policies.
- Supervise staff members involved in resident care, providing leadership and guidance to ensure high standards of service delivery.
- Oversee medication administration processes, ensuring accuracy and safety in compliance with regulations.
- Collaborate with healthcare professionals, families, and residents to address concerns and adjust care strategies accordingly.
- Maintain documentation related to resident care.
- Ensure adherence to safety protocols and quality standards within the facility
- Provide training and support for staff on topics such as dementia care, memory care techniques, and resident engagement strategies.
- Monitor resident progress and adjust care plans as necessary to meet evolving needs.
Experience
- Proven experience in assisted living facilities, nursing homes, or long-term care environments.
- Extensive background working with seniors, including those with developmental disabilities or requiring specialized memory or dementia care.
- Demonstrated supervisory or management experience within healthcare or social service settings.
- Pennsylvania Certification in medication administration or LPN license
- Strong leadership skills with the ability to manage teams effectively while maintaining a compassionate approach.
