Starting salary: $43,000, annually

The New Hampshire Harm Reduction Coalition (NHHRC) is seeking a full-time Syringe Service Program (SSP) Care Coordinator to join our team. This is a community-based position focused on building supportive, strengths based relationships with individuals who use drugs and connecting participants with health care, behavioral health, substance use, and other community services.

The position will primarily serve the Strafford County region, including Dover, Somersworth, Rochester, and Portsmouth, while providing coverage in other NHHRC service areas, including Concord and Manchester, as needed to support program operations and continuity of services.

About the Position

The Care Coordinator works directly with SSP participants to identify individual needs and goals and provides referrals, navigation, follow up, education, and other support. The position also develops relationships with health care providers, social service agencies, treatment providers, recovery organizations, and other community partners to improve access to services and reduce barriers experienced by people who use drugs.

This position is primarily community based and requires regular travel, flexibility, strong interpersonal skills, and the ability to work effectively with individuals experiencing complex health and social needs.

Key Responsibilities

● Develop and maintain relationships with local and statewide health, behavioral health, substance use, and social service providers and community based organizations.

● Work with community partners to identify and reduce barriers that prevent people who use drugs from accessing needed services.

● Regularly participate in harm reduction outreach services within the assigned region and provide coverage in other NHHRC service areas as needed.

● Work with participants to support self identified goals related to personal safety, physical health, mental health, and overall well being.

● Provide referrals, care navigation, and follow up for services including primary and specialty health care, HIV/HCV/STI testing, mental health services, substance use care, and other community resources.

● Provide education and support related to overdose prevention and reversal, infectious disease prevention, safer practices, wound and vein care, medications used to treat substance use disorder, and sexual health.

● Connect participants with appropriate prevention services, including vaccinations, PrEP, PEP, and other infectious disease prevention resources.

● Assist with participant documentation, service data entry, identification cards, educational materials, and program supplies.

● Participate in regional and statewide collaborative meetings, task forces, and community partnerships.

● Build and maintain relationships with key stakeholders and community agencies.

● Contribute to ongoing program development, policies, and procedures consistent with NHHRC's mission and values.

● Maintain timely and accurate program documentation.

● Work collaboratively with NHHRC staff and SSP volunteers.

Minimum Qualifications

● 1 years of documented work experience supporting adults with serious mental illness and/or substance use disorder.

● Effective interpersonal skills and demonstrated ability to work respectfully with vulnerable populations.

● Excellent communication, organization, planning, and time-management skills.

● Proficiency with computers and Google WorkSpace applications, with the ability to learn new systems and applications.

● Ability to maintain participant confidentiality and professional boundaries.

● Ability to work independently in community based settings while also functioning effectively as part of a team.

Preferred Qualifications

● Knowledge of community health networks within the region.

● Knowledge of local and regional social service agencies and assistance programs, including housing, nutrition, Medicaid, SNAP, SSI/SSDI, fuel assistance, and related resources.

● Existing professional relationships or networking connections with regional health and social service organizations.

● Experience working with people who use drugs and individuals experiencing complex health, behavioral health, housing, or social service needs.

Position Requirements

This position is primarily community based rather than office based. The successful candidate must be willing and able to:

● Regularly travel throughout the assigned region and to other areas of New Hampshire as needed.

● Work a flexible schedule based on program and participant needs.

● Work respectfully with people who use drugs while supporting bodily autonomy and individual self-determination.

● Maintain a valid New Hampshire driver's license and reliable transportation.
