What are the responsibilities and job description for the Adoption Coordinator (Bachelor's Degree Required) position at New Foundations, Inc.?
The Adoption Coordinator is responsible for preparing and facilitating adoptions and adoptive support services for children and families contracted for service through the Philadelphia Department of Human Services and the State Wide Adoption Network. The Adoption Coordinator is under the direct supervision of the Adoption Supervisor. CURRENT Pennsylvania (or Residing State) Driver’s License, Auto Insurance, and acceptable PA State Police, Child Abuse Clearances and FBI clearances are required.
- Completes child and family profiles in accordance with county and SWAN regulations and timelines.
- Completes home studies, background checks and related documentation in accordance with county and SWAN regulations and timelines.
- Participates in monthly face-to-face Supervision with the Adoption Supervisor, and ongoing communication via phone or email as needed to discuss case concerns.
- Generates adoptive files and records which may include confidential information.
- Utilizes computer programs such as Word, Excel, Access, EM, Internet and Email.
- Prepares invoices and completes SWAN benchmarks in a timely manner.
- Completes inter-agency and county file reviews in a timely manner.
- Completes child preparation activities for children referred for Child Preparation Services.
- Attends all inter-agency, county and SWAN meetings and training.
- Completes finalization reports and paperwork and attends adoption finalization hearings.
- Conducts pre-adoptive training in accordance with agency regulations.
