Join our caring and committed team as a PRN RN and make a significant difference. In this important role, you'll assist our Care Coordinator with quarterly home visits, directly influencing our clients' well-being. You will be essential in providing compassionate nursing care, supporting patient recovery, and enhancing overall health. Your skills will help us uphold high standards in patient outcomes while creating a positive, supportive environment for both patients and staff. Please NOTE: This position is "as needed"; most appointments will be scheduled in advance.
Responsibilities may include
- Performing comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation.
- Monitor patients' conditions directly within their homes.
- Coordinate care plans with multidisciplinary teams; conduct triage assessments and case management to optimize patient outcomes.
- Document all patient interactions thoroughly in compliance with HIPAA regulations and medical documentation standards.
- Provide education on disease management or post-discharge care tailored to diverse populations, including geriatrics, behavioral health patients, or individuals with developmental disabilities.
Skills
- Proficiency in specialized nursing procedures, including Botulinum toxin treatment administration and sterile processing techniques.
- Strong knowledge of medical terminology, anatomy, and physiology.
- Skilled in managing complex cases involving clients confined to their beds.
- Ability to perform advanced patient assessments.
- Experience working with diverse populations, familiarity with geriatrics, and dementia care are advantageous.
- Competence in handling emergency situations through basic life support (BLS), airway management, ventilator support, and acute care protocols.
- Knowledge of infection control practices; experience with infectious disease care is a plus.
- Able to work independently in clients' homes across southwest Michigan.
