Essential Job Functions:
- Provides direct daily shift coverage in program, follow scheduled assignments away from the facilities of NEV and implement program routines to assist participants to access community services and resources, with the goal of being as independent as possible.
- Work collaboratively with area community organizations, employers and service professionals.
- Assist with writing individualized, person-centered plans that focus on career exploration, personal objectives and interests.
- Assist program participants towards career exploration, educational, social, recreational resources and volunteer opportunities in line with PCP process and ISP goals.
- Assist program participants to develop new skills/tasks (career exploration, private transportation, health and wellness, life skills i.e. cooking).
- Assist with assessing individual needs and teach classes to program participants following established curriculum and monitors effectiveness.
- Assist program participants to obtain training or non-paid work-based learning opportunities.
- Provide accommodations, training, support and integration for program participants in the community.
- Establish, maintain and promote excellent working relationships among participants, community sites and employers.
- Maintain informal and formal communications with all program staff and provides written communications including progress notes, data sheets and Icident Reports; prepare reports detailing activities, progress and future needs/interests.
- Participates in the development and is responsible for direct implementation of the ISP, participates in writing annual assessments and goals and attend team meetings as requested.
- As needed, provides advocacy and case management services; work collaboratively and communicate effectively with others involved in person’s life (i.e. families, residential, service coordinators).
- Protects the rights and dignity and safeguards confidentiality of program participants.

- High school diploma required. Associate’s degree preferred
- 0-2 year's experience
- Strong communication skills, verbal and written
- Exceptional teaching and organizational skills
- Knowledge of community resources and strong research skills
- Ability to represent program participants and program services in a professional manner
- Ability to travel extensively in the community and to teach others to use public transportation (when available and deemed appropriate).
- Familiar with disability-related issues, services/resources, accommodations and assistive technology
- Proficient computer
