Manage a weekend caseload within your local territory Conduct skilled nursing visits, assessments, and follow-ups Develop and adjust personalized care plans Partner with physicians and care teams to ensure continuity of care Educate patients and families to promote independence and safety at home ------------------------------------------------------------------------------------------------------------ What You’ll Need Active RN license (CT) 1 year of nursing experience (Home health preferred not required) Willingness to travel locally for patient visits Valid driver’s license, reliable vehicle & insurance Live within the service area Lift, pull, and push 50-100lbs Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
