Job Title: Home Based Crisis Interventionist

Department: Home Based Crisis Intervention (HBCI)

Position Summary

The HBCI Interventionist provides intensive, short-term crisis intervention and stabilization services to children, youth, and their families in both Erie and Niagara Counties. Services are designed to address clinical and family-system needs, ensure safety within the home, reduce crises, and prevent unnecessary psychiatric hospitalization or residential placement. Interventionists utilize trauma-informed, evidence-based, person-centered practices to support the child and family as a whole.

Department

New Directions’ Home Based Crisis Intervention Program delivers time-limited, high-intensity in-home support to families experiencing acute behavioral or emotional crises. The program’s goal is to stabilize the family, strengthen functioning, and keep youth safely in their homes and communities.

Pay and Benefits

  • Starting Pay: $58,548.00 – $59,865.33 annually (based on education and relevant experience)
  • Paid Time Off: 3 weeks PTO in the first year

Professional Development Opportunities

New Directions invests in developing future leaders through clinical supervision, agency-sponsored trainings, continuing education opportunities, and internal advancement pathways.

Position Specific Duties/Responsibilities

  • Maintain a caseload of 2–3 families for program-defined timeframes (4–6 weeks or 6–9 weeks).
  • Provide daily meetings with child/youth and family for at least one hour per session.
  • Participate in crisis on-call rotation; provide in-person crisis support when necessary.
  • Work in both Erie and Niagara Counties to meet family needs.
  • Create individualized safety/crisis plans and treatment plans with each family.
  • Collaborate with multidisciplinary teams, including SPOA and community providers.
  • Use evidence-based, trauma-informed, person-centered practices.
  • Maintain knowledge of community resources in both counties.
  • Attend required trainings and scheduled individual and group supervision.
  • Provide accurate and timely documentation in the agency’s EHR and CAIRS system (admission, discharge, 1-month, and 3-month post-discharge).
  • Maintain confidentiality and comply with all program and agency policies.
  • Perform other duties as assigned by the HBCI Supervisor.


Knowledge, Skills, And Abilities

  • Ability to work effectively with youth, families, staff, and community partners from diverse backgrounds.
  • Skilled in supporting youth with behavioral, emotional, or mental health challenges.
  • Strong communication, organizational, and documentation skills.
  • Ability to work as part of a team and accept supervision.
  • Flexibility and adaptability to changing family needs and schedules.
  • Ability to communicate professionally across multiple mediums (email, written notes, phone, etc.).
  • Valid NYS driver’s license, adequate auto insurance, and reliable transportation.
  • Ability to pass annual physical for Category 1 job classification.
  • Compliance with all agency safety and health standards.


Qualifications

  • Master’s degree and license(preferred), or
  • Bachelor’s degree with at least one year of relevant experience in community-based mental health or case management may be considered.
