Job Summary
We are seeking a dedicated and compassionate Case Manager specializing in Mental Health to join our dynamic team. Key Responsibilities:Clinical & Service Coordination:
- Provide case management services, including assessment of client needs, service planning, care coordination, and linkage to community resources.
- Monitor client progress and ensure services are aligned with individualized treatment goals.
- Maintain consistent communication with clients, families, and interdisciplinary team members.
Staff Oversight & Productivity Management:
- Monitor and manage assigned team members (e.g., QMHPs, support staff) to ensure clients are seen according to authorized service hours.
- Track staff productivity and service delivery to ensure compliance with billing and Medicaid requirements.
- Review staff schedules and caseloads to ensure appropriate service distribution and utilization of hours.
- Ensure all team members complete and submit required documentation accurately and within established timeframes.
- Provide ongoing support, accountability, and corrective guidance to staff to address performance concerns related to attendance, productivity, or documentation compliance.
- Collaborate with leadership to implement performance improvement strategies as needed.
Documentation & Compliance:
- Ensure all clinical documentation meets Medicaid, licensing, and agency standards for accuracy, timeliness, and medical necessity.
- Conduct regular audits of staff documentation to ensure compliance and provide feedback for improvement.
- Maintain up-to-date records for assigned clients and ensure all required reports are completed.
Marketing & Referral Development:
- Actively engage in community outreach efforts to establish and maintain relationships with referral sources, including schools, DSS/CPS, courts, healthcare providers, and community organizations.
- Promote agency services to increase client referrals and program census growth.
- Track and follow up on referrals to ensure timely intake and service initiation.
- Identify new opportunities for program expansion and community partnerships.
Collaboration & Team Leadership:
- Participate in team meetings, case staffing, and supervision as required.
- Serve as a liaison between clinical staff, administration, and external stakeholders.
- Support a culture of accountability, professionalism, and high-quality service delivery.
Qualifications:
- Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, Human Services, or related field (Master’s preferred).
- Relevant experience in community mental health, case management, or intensive in-home services.
- QMHP Certification or LMHP License
- Experience with staff supervision/management, productivity management, or team coordination strongly preferred.
- Knowledge of Medicaid documentation standards and service requirements.
- Strong organizational, leadership, and problem-solving skills.
- Demonstrated ability to market services and develop referral sources.
- Excellent communication and interpersonal skills.
Pay: $24.40 - $28.00 per hour
