Case Manager
Peninsula Healthcare Connection is seeking a Case Manager, with a Bachelors in Social Work or related field, to make $65,000-75,000 annually, dependent on years experience, as we try to change the experience of the unhoused and medically complex in Santa Clara County.
Case Manager Job Summary
Case Managers undertake a range of key responsibilities for clients that relate to healthcare, behavioral health, and housing. Case Managers will coordinate care plans for clients and manage delivery of services with community partners. At Peninsula Healthcare, Case Managers will meet with clients at a determined rate of need and work with clients on personal stability, increased self-sufficiency, improve health and overall well-being.

Case Manager Requirements:
- BSW or degree in related field, AA units totaling 75 semester or 120 quarter units acceptable instead of Bachelors.
- Minimum two years’ experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
- Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds.
- Valid CA driver’s license, reliable transportation, and legally required insurance.
Some Case Manager job duties:
- Independently manage assigned caseload.
- Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.
- In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals.
- Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
- Support team with crisis intervention for immediate housing needs, medical, mental health and substance use issues.
