What You’ll Do
- Perform INR draws and medication management support
- Set up and assist with medication organization
- Serve as a liaison between clients and physicians
- Complete patient assessments and accurate documentation
- Supervise Personal Care Assistants (PCAs)
- Support coordinated, patient-centered care plans
- Ensure continuity of care and communication across providers
- Active Registered Nurse (RN) license required
- Strong assessment and clinical judgment skills
- Highly organized and detail-oriented
- Ability to work independently in the field
- Strong communication and interpersonal skills
- Compassionate, patient-focused approach to care
- Flexible weekday schedule (8:00 AM – 4:30 PM)
