What You’ll Do
- Evaluate clients and assess physical therapy needs
- Develop and implement individualized treatment plans
- Help clients restore function, mobility, strength, and independence
- Provide therapeutic exercises and rehabilitation services
- Educate clients and caregivers on treatment plans and home exercises
- Monitor progress and adjust treatment plans as needed
- Maintain accurate and timely documentation
- Collaborate with healthcare providers and care teams to support optimal client outcomes
- Current Physical Therapist license required
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-focused approach to care
- Ability to work independently and manage time effectively
- Home health experience preferred, but not required
