Targeted Youth Case Manager

Responsibilities

Provide targeted case management services for youth who are determined eligible as seriously emotionally disturbed per ARM 37.83.805. Facilitate the provision of community services appropriate and needed by the youth and family and ensure appropriate referrals for linkage services are available and used. Required to maintain regular contact with the youth, parent/guardian, family, and treatment team through in person visits or phone calls.

Education And/Or Experience

A minimum of a bachelor’s degree in human services field with at least one year of full-time experience serving people with mental illness. Individuals with other educational backgrounds, who as providers, consumers, or advocates of mental health services have developed the necessary skills may also qualify. Must be skilled in interviewing to gather data and completing needs assessments in preparation of narratives/report development of case management plans. Possess the basic knowledge of behavior management techniques, emotional and behavioral disorders, and individual and group communications. Have a basic understanding of Child Welfare Laws, including the Indian Child Welfare Act (ICWA) and be able to process for compliance.

Major Functions

• 1. Develop assessment of participant(s) to carry out the strengths-based service plan. The assessment is to identify the strengths, abilities, potentials and skills of the participant and the participant's family. The assessment will identify resources and services needed to carry out the service plan. Work closely with community-based advocacy groups and other agency childcare staff to promote and/or advise the necessity for service.
• 2. ???Maintain progress notes for each participant. The progress notes must be entered into the participant's clinical record at least every 30 days, and upon the occurrence of any significant change in the participant's condition.
• 3. ???Develop and maintain a current crisis response plan for participant(s) and family with the treatment team. Perform crisis intervention as needed and advocate on behalf of participant and family. When needed, make presentations to managed care clinicians, community and agency representatives and advocate for wrap-around care, out of home placement and/or higher levels of care.
• ???4. Develop and implement an active discharge plan in the early stages of targeted youth case management.
  
  

Plan for discharges in a way that affirms the progress and abilities of the participant and family to manage the treatment needs of the participant by establishing measurable discharge criteria.

Review discharge criteria monthly and assess the need for continuation of services.

• 5. Provide access to resources to meet the needs of the participant, which may include, but is not limited to monitoring and assessing the impact of services being provided, ensuring that services identified in the case plan are being provided, making appropriate referrals, including to advocacy organizations and service providers.
• 6. ???Assist the participant and family in assessing natural family support and community activities to develop a support network with increased social and recreational functioning for self-reliance and healthy living. Empower the family in developing the abilities to manage their own treatment.
• 7. Maintain in person contact with participant(s) on a periodic basis. A minimum of one face-to-face visit is required per month, except participant(s) outside of a 100-mile radius of the Mental Health Facility Office. In this instance, contact can be made via phone.
• 8. No less than 50% of case management contact with participant(s) must be outside of the Mental Health Facility Office. All contacts with the participant(s) must be in a place that is convenient for the participant(s).
• 9. Other duties as assigned by Supervisor and New Day, Inc. administration.
