Position Summary


At New Day Assisted Living, the Registered Nurse (RN) serves as a clinical leader and consultant, supporting our mission to provide compassionate, person-centered care to those that may have mental and physical disabilities as well as individuals living with dementia.


The RN ensures that residents receive safe, appropriate care while maintaining compliance with standards set by the Wisconsin Department of Health Services (DHS). This role emphasizes clinical oversight, staff education, and quality of life, rather than continuous bedside nursing.


The RN position entails working full time, typically Monday-Friday, 8am – 5pm. At times you may need to be available earlier in the morning, later in the evening, and occasionally you may need to offer flexibility on weekend days.


The RN position serves multiple New Day Assisted Living locations. Daily worksite will be determined based on resident and community needs and flexibility is required for short-notice adjustment. The RN position is required to participate in new resident preadmission assessments which often requires assessments to be conducted off-site at a healthcare facility or in a private residence.


This position requires you to be on-call for your assigned communities and available to coach staff in demanding situations.

Key Responsibilities


Clinical Oversight & Resident Wellness

  • Complete and review resident assessments as required
  • Monitor residents for changes in physical, cognitive, or behavioral condition
  • Ensure service plans reflect current needs, preferences, and safety considerations in accordance with DHS 83 requirements and company policy
  • Provide guidance on dementia-related behaviors and non-pharmacological interventions
  • Reports any abuse, suspected abuse, or alleged abuse to the appropriate person/agency

Medication & Health Services Oversight

  • Oversee medication administration systems to ensure compliance with Wisconsin CBRF regulations (DHS 83)
  • Train and validate competency of caregivers administering medications
  • Monitor for:
    • Adverse reactions
    • Medications Effectiveness
    • Medication errors and follow-up actions
  • Coordinate with pharmacies, prescribers, and hospice providers, and all other health services vendors

Delegation, Training & Staff Support

  • Delegate nursing tasks in accordance with Wisconsin Nurse Practice Act
  • Provide hands-on and ongoing staff training, including but not limited to:
    • Medication administration
    • Blood glucose monitoring
    • Recognition of changes in condition
    • Dementia-specific care approaches
  • Serve as a clinical resource for caregivers and leadership
  • Evaluate and document staff competency for delegated tasks

Care Coordination & Communication

  • Identifies self as an important part of the interdisciplinary team and seeks a collaborative environment amongst disciplines and other staff members
  • Collaborate with physicians, nurse practitioners, hospice, and other providers
  • Communicate proactively with families regarding resident condition changes
  • Participate in care conferences and service planning meetings
  • Assist with hospital discharges and transitions back to the facility
  • Assist with new resident admissions

Regulatory Compliance & Documentation

  • Ensure compliance with all applicable CBRF regulations through the Wisconsin Department of Health Services
  • Maintain accurate and timely clinical documentation
  • Assist with state surveys, audits, and plans of correction
  • Support development and review of clinical policies and procedures

Quality Assurance & Risk Management

  • Monitor and review:
    • Incidents and accidents
    • Falls and injury trends
    • Infections and infection control practices
  • Participate in quality improvement initiatives
  • Conduct regular quality assurance reviews of staff documentation and medication administration
  • Recommend system improvements to enhance resident safety and care quality

Duties listed are representative and not exhaustive. Other duties may be assigned to support organizational, community, or resident needs. All nursing activities must align with Wisconsin DHS regulations and current best practices.


Requirements:

Qualifications:


Required

  • A desire to work with the elderly in an environment that upholds their privacy, dignity, choice, independence, and individuality
  • Active Registered Nurse (RN) license in the State of Wisconsin
  • Knowledge of Wisconsin CBRF regulations (DHS 83)
  • Experience in geriatrics, assisted living, or long-term care
  • Strong assessment, delegation, and communication skills

Preferred

  • Experience in memory care or dementia care
  • Experience in a supervisory or nurse consultant role
  • Familiarity with electronic health record systems

Work Schedule

  • Monday – Friday 8am-5:00pm with nights, weekends and holidays as needed
  • 24/7 on call availability for clinical concerns

Physical Requirements

  • Ability to assess and observe residents effectively
  • Occasional lifting, bending, and assisting residents if needed
  • Ability to respond promptly to urgent situations
