Direct Support Professional

Direct Support Professionals assist clients with all the activities of daily living, personal care,

and program implementation. Responsible for the daily well-being and safety of the clients,

while ensuring adequate supervision. Assist with household maintenance and housekeeping

and is responsible for various tasks to support.

A. Assist client with daily care, medical needs and support, ADLs and mental health support.

B. Clocking in and out daily on work daily app or EVV if required.

C. Charting medication, Behaviors, Case noting daily. Picking up groceries and medication,

D. Assisting client with cooking, cleaning, errands, appointments, arranging transportation and

providing transportation and other duties as directed.

E. Identifying and researching available resources and activities in the community for residents.

F. Creating a trusting, friendly and comfortable professional relationship, and environment with

residents. Work harmoniously and show respect to all internal and external individuals.

G. Demonstrate an understanding of the person as a unique individual and how to implement

programs by the Coordinated Service and Support Plan (CSSP), Coordinated Service and

Support Plan Addendum (CSSP-A), and Individual Abuse Prevention Plan.

H. Provide person-centered services demonstrated through practice, action, and language.

I. Assist with daily housekeeping and household maintenance.

J. Ensure adequate supervision of client including remaining on shift until a replacement

employee arrives. Collaborates with the team to complete other duties as assigned or as

needed.

K. Complete all mandatory training determined by New creation or 144G regulations. Read and

follow company policies and procedures.

L. Documents program data and daily log information accurately and in a timely manner.

M. Administer medication following medication administration guidelines.
