As a direct care professional with responsibility for scheduling medical appointments and ensuring safe transportation to and from these appointments using company vehicles, you will play a vital role in supporting the goals and objectives of each resident's individual service plan.

The Role:
- Schedule and confirm resident appointments with medical providers
- Transport residents to their appointments and communicate their needs to the provider when necessary
- Evaluate the need for follow-up appointments and schedule them accordingly
- Effectively communicate doctor's orders to relevant personnel
- Maintain thorough documentation and ensure timely completion
- Provide personal care assistance when required
- A high school diploma or equivalent is required
- Must possess an active driver's license with a clean driving record
- Prior experience working with elderly or disabled populations is preferred but not essential
