What are the responsibilities and job description for the LPN position at New Beginnings Adult Day Care Services, LLC?
Qualifications:
Shall be a licensed LPN with valid state credentials and a minimum of 1 year applicable experience (Full time or equivalent) in working with the aged and disabled. (The credentials specified in the description are those preferred.)
Responsibilities and Functions:
a. Must be able to function with minimal direct supervision.
b. Conforms to the Adult Day Care facility policy and procedures.
c. Maintains confidentiality relative to patient care and New Beginnings practices.
d. Communicates information effectively to appropriate personnel to facilitate continuity of care or report changes in patient’s condition.
e. Accepts accountability for own practices.
f. Maintains competency by participating in continuing educational programs and meets states specific requirements.
g. Assist with Quality Improvement/Utilization Review Activities to ensure that outcomes are achieved and maintained.
h. Seek input from health team members regarding POC to obtain knowledge and support.
i. Obtain vital signs, weight (if needed), and obtain history of client and current problem areas.
j. Participate in orientation and in-service training.
k. Notify physician or liaison of any changes in client’s status.
l. Assist staff with the use of standard protocols for communicable diseases and infection control.
m. Hold educational in-services for staff and participants
n. Assist supervisor with the oversight of participant Health Records.
o. Assist with the oversight of client taking their own medications.
p. Perform other duties as assigned.
