New Age Hospice is currently hiring for a Full-Time RN in San Antonio, TX and surrounding areas.
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our dedicated team. In this role, you will coordinate and provide high-quality, patient-centered hospice care to individuals and families facing end-of-life challenges. You will manage a caseload of patients, ensuring their physical, emotional, and spiritual needs are met with dignity and respect.

Responsibilities:
- Manage a caseload of hospice patients in home, facility, or inpatient settings.
- Conduct initial and ongoing assessments of patient and family needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, symptom management, and emotional support.
- Educate patients and families on disease progression, medications, and care techniques.
- Coordinate with physicians, social workers, chaplains, and other team members.
- Participate in on-call rotation as needed.
- Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
Requirements
- Current RN license in TX and/or Compact State
- Minimum of 1 year of nursing experience; hospice or palliative care preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Valid driver’s license and reliable transportation
- EMR experience preferred (prefer WellSky or Kinnser)
