What are the responsibilities and job description for the Case Manager position at Networks Connect Healthcare Staffing?
About the Opportunity
We’ve partnered with a leading pediatric health system in the Washington, D.C. area to fill three Case Manager (RN) openings on a tenured, well-supported team. This is a strong opportunity for experienced nurses who want a true Monday–Friday day-shift schedule, meaningful work with patients and families, and the chance to grow into pediatric case management with robust training.
Our client is:
- A leading pediatric health system in the Washington, D.C. metro area
- Magnet®-designated for nursing excellence
- Nationally recognized for pediatric specialty care and clinical excellence
- Committed to professional nursing practice, shared leadership, and continuous improvement
If you’re an experienced RN looking for a true M–F schedule and a step into a specialty-leading environment, this is one of the strongest case management opportunities in the region.
Why This Role Stands Out
- Monday–Friday, day shift — no nights, no rotating shifts
- Salaried/exempt — with a light on-call rotation (weekend coverage 3–5 times per year, plus a minor/major holiday rotation)
- Robust training provided — strong acute-care RNs without prior case management or pediatric experience are encouraged to apply
- Multiple openings — three Case Manager vacancies on the same team, so you’d be onboarding alongside peers
- A team that values critical thinking, autonomy, transparency, and the ability to anticipate patient needs
The Role
As a Case Manager, you’ll provide full-scope case management services to patients and their families — including assessment, discharge and transitional planning, care coordination, utilization review, and community outreach. You’ll partner with the multidisciplinary care team to identify discharge barriers early, build effective plans of care, and ensure smooth transitions to external providers and services.
This is a role with both clinical depth and operational impact. You’ll work cross-functionally to manage financial risk to families, navigate payer authorizations, and contribute to performance improvement initiatives that strengthen patient outcomes.
What You’ll Own
- Initial patient and family assessments to identify discharge barriers and care needs
- Development and execution of discharge and transitional plans of care in partnership with the multidisciplinary team
- Clinical reviews and utilization management using nationally recognized criteria
- Payer coordination — authorizations, denials, and communication around level-of-care changes to limit financial risk to families
- Documentation in accordance with departmental policy and regulatory standards
- Participation in shared leadership, performance improvement initiatives, and community engagement
What You’ll Bring
- Active RN license in the District of Columbia (or eligible to obtain prior to start)
- Bachelor’s degree required (Master’s preferred)
- 5 years of nursing experience with at least 3 years in an acute-care setting
- Pediatric and case management experience are helpful but not required — the team provides robust training
- Strong critical thinking, analytical skills, time management, and ability to work autonomously
- Comfort with transparency, flexibility, and anticipating patient needs based on clinical pathways
- Certification in Case Management or Utilization Management preferred
The Details
- Compensation: $85,000–$96,000 based on experience
- Schedule: Monday–Friday, day shift, salary/exempt
- On-call: Weekend rotation 3–5 times per year, plus a minor/major holiday rotation
- Location: Washington, D.C. area — onsite
