Overview:
Network Solutions IPA is hiring a full-time Nutritionist to join our Care Management Program. You ll support Medicaid Members by addressing nutrition-related health needs as part of our Social Care Network (SCN) partnership.
Key Duties:
- Assess Members nutritional needs and develop care plans.
- Coordinate nutrition services like food prescriptions or pantry stocking.
- Collaborate with care teams to ensure Member support.
Qualifications:
- Registered Dietitian Nutritionist (RDN) or Certified Nutrition Specialist (CNS) with NY licensure (or eligibility).
- 2 years of nutrition experience, ideally with underserved populations.
- Comfort with IT systems and strong communication skills.
