Nutritionist

Overview:

Network Solutions IPA is hiring a full-time Nutritionist to join our Care Management Program. You’ll support Medicaid Members by addressing nutrition-related health needs as part of our Social Care Network (SCN) partnership.

Key Duties:

• Assess Members’ nutritional needs and develop care plans.
• Coordinate nutrition services like food prescriptions or pantry stocking.
• Collaborate with care teams to ensure Member support.

Qualifications:

• Registered Dietitian Nutritionist (RDN) or Certified Nutrition Specialist (CNS) with NY licensure (or eligibility).
• 2 years of nutrition experience, ideally with underserved populations.
• Comfort with IT systems and strong communication skills.
