Position Summary
The Clinical Case Manager fulfills a crucial at Network by providing wrap around support and removing/assisting with barriers for neighbors seeking mental health services. It is expected that the CCM will implement trauma-informed, person centered, direct case management services to neighbors. Conducts face-to-face intakes with neighbors with mental health needs. Provides ongoing case management to neighbors who require more intensive case management services. Establishes rapport with neighbors by utilizing empathetic listening and assesses level of client risk by asking open-ended questions and applying active listening skills. Serves as back-up support to Network staff by provided de-escalation, crisis intervention, safety planning, and information/referrals to individuals accessing Network services.
Key Responsibilities:
- Conducts face-to-face intakes for new neighbors experiencing mental health symptoms and/or identifies mental health assistance as a need.
- Completes psychosocial assessment and self-sufficiency scale to gain neighbor’s history and better identify needs
- Assesses individual for mental health crisis to ensure safety or if a higher level of care is needed
- Collaborates with Clinicians to refer neighbors to counseling services and assists Clinicians in administrative tasks (contacting neighbors regarding appointments, checking them in, etc.)
- Review neighbors’ PHQ9s and GAD7s with Clinicians to screen for depression and anxiety
- Conducts individualized safety plans and suicide risk assessments with neighbors endorsing past/current suicidal ideation.
- Coordinates services with staff to effectively provide a continuum of care for all of the neighbor’s needs.
- Assures that client records and forms are accurate and current; assists in providing statistical information as required.
- Enters all data into Network’s HIPPA compliant platform accurately and within 48 hours of collection
- Maintains appropriate professional boundaries with clients.
- Assists with special events and activities scheduled during the week.
- Attends required staff meetings and in-service training sessions.
- Performs basic administrative tasks, including reports and statistical summaries, and other duties, not listed here, deemed necessary for the well-being of the agency.
- Provides crisis telephone counseling, establishes rapport, and assesses level of client risk and as well as providing mental and emotional state of mind by active listening, comforting words and positive problem solving.
Other Duties:
- Maintains current professional knowledge of mental health, trauma informed care, substance use, harm reduction, etc.
- Performs other duties, as assigned.
Qualifications
Knowledge, Skills and Abilities:
- Knowledge of the dynamics of poverty and how it impacts children, women, and men
- Ability to deal calmly and effectively in high stress situations.
- Ability to be flexible and adapt as situations change.
- Ability to take thorough and accurate case notes.
- Ability to work as a team member with other staff members.
- Ability to interact well with diverse populations.
- Ability to provide appropriate interventions and referrals for each individual.
- Ability to maintain confidentiality of records relating to clients' services.
- Ability to read and interpret documents such as standard operating procedures.
- Ability to read, write and converse in English, Spanish a plus.
- Experience with Microsoft office applications (Outlook, Word, Excel, OneDrive, etc.)
Licenses and Certifications:
Valid Texas Driver’s License
Preferred: Mental Health Certification - Qualified Mental Health Professional (QMHP), Licensed Social Worker (BSW or MSW), Community Health Worker (CHW), etc.
Qualifications:
Bachelor’s Degree in Social Work (Masters degree preferred), Psychology, Counseling, or related Human Services field. One to two years’ experience in crisis intervention and mental health services preferred.
Mental and Physical Duties:
- Must have emotional and physical stamina to tolerate prolonged sitting or standing to deal with a variety of stressful situations, including responses to complaints, difficult requests from programs and individuals in crisis, and internal and external interactions, while maintaining a sense of humor and composure.
- Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.
- Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) frequently.
