Travel RN Care Coordination (Director/Manager) to join our healthcare leadership team for a 13-week travel assignment in Greenbrae, California. The ideal candidate is a strong, collaborative healthcare leader with extensive knowledge of care coordination, discharge planning, and utilization management in an acute care setting.
As the Care Coordination Director/Manager, you will oversee the daily operations of the Care Coordination and Social Services Department, ensuring compliance with hospital policies, regulatory standards, and best practices to deliver high-quality, patient-centered care.

Key Responsibilities
- Provide strategic and operational leadership for the Care Coordination team, including Case Managers and Social Workers.
- Develop and implement effective care transition processes and discharge planning strategies.
- Ensure efficient utilization management, patient throughput, and timely discharge processes.
- Collaborate with multidisciplinary teams to coordinate patient care and improve outcomes.
- Monitor compliance with CMS, Joint Commission, and hospital guidelines.
- Lead and mentor staff, fostering professional growth and team engagement.
- Analyze data, track metrics, and report departmental performance to hospital leadership.
- Manage departmental budgets, staffing, and workflow efficiency.
Qualifications
- Active CA RN License.
- Minimum 5–7 years of hospital Inpatient case management or care coordination experience, with 3 years in a leadership role (Manager, Director, or Supervisor).
- Strong understanding of utilization review, discharge planning, and patient flow management.
- Excellent communication, leadership, and organizational skills.
- Experience in acute care hospital settings required.
- BLS Certification (AHA) preferred.
