seeking a compassionate and experienced Medical Social Worker (LMSW/LCSW) to join its Care Management team. This is a permanent, direct-hire opportunity with one of New York's most respected healthcare organizations.
Key Responsibilities:
✔ Complete comprehensive psychosocial assessments upon patient admission
✔ Identify high-risk patients and coordinate appropriate discharge planning services
✔ Collaborate with multidisciplinary healthcare teams to ensure seamless patient care transitions
✔ Coordinate aftercare services and patient/caregiver education
✔ Assist with readmission prevention and length-of-stay reduction initiatives
✔ Advocate for patients and families while addressing social, emotional, and resource-related needs
✔ Maintain accurate documentation and compliance with departmental standards
Required Qualifications:
✅ Master's Degree in Social Work (MSW) from an accredited program
✅ Current New York State LMSW or LCSW License
✅ NYS SCREEN Certification (or ability to obtain within 3 months of hire)
✅ Strong communication, assessment, and care coordination skills
Preferred Experience:
⭐ 2 years of acute care hospital or healthcare-related social work experience
⭐ Experience with discharge planning and transitional care management
⭐ Ability to thrive in a fast-paced, collaborative healthcare environment
