Job Summary and Qualifications:
As a Case Manager, you will guide patients and families throughout their care journey. You will coordinate services, connect them with resources, and create care plans tailored to each patient’s needs. Working closely with physicians, nurses, and department leaders, you will support safe transitions, clear communication, and high-quality care across the continuum.
Required Qualifications:
- 2 years of Recent (within the last 3 years) Acute Care Experience. Looking for ICU, CCU, Med-Surg or LTAC Case Management experience.

- Associate degree, or Certificate
