Responsibilities
- Perform thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols.
- Manage complex clinical procedures such as catheterization, suturing, tube feeding, ventilator management, and ostomy care with precision.
- Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Coordinate discharge planning, case management, and follow-up care to facilitate smooth transitions across care settings.
- Respond promptly to acute pain episodes, triage patient concerns, and provide appropriate interventions in emergency situations such as trauma or infectious disease outbreaks.
- Collaborate with multidisciplinary teams including physicians, therapists, and social workers to develop personalized care plans aligned with NCQA standards and ICD coding requirements.
