Job Title: Nurse Practitioner - Nephrology
We are a dedicated nephrology clinic seeking a compassionate and skilled Nurse Practitioner to join our team. In this role, you will provide high-quality, patient-centered care to individuals with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, electrolyte imbalances, and other kidney-related conditions. You will work collaboratively with our nephrologists and multidisciplinary team to manage outpatient care, promote kidney health, and improve patient outcomes. Experience in dialysis is a plus and will be highly valued.
Key Responsibilities
- Conduct comprehensive patient assessments, including history taking, physical examinations, and review of diagnostic tests (e.g., labs, imaging).
- Diagnose and manage acute and chronic kidney conditions, including CKD progression, ESRD, glomerulonephritis, kidney stones, and related comorbidities such as hypertension and anemia.
- Develop, implement, and evaluate individualized treatment plans, including prescribing medications, ordering therapies, and recommending lifestyle modifications.
- Provide patient education on kidney disease management, dialysis options (if applicable), diet, fluid restrictions, and medication adherence.
- Collaborate with nephrologists, nurses, dietitians, and social workers to coordinate care and ensure seamless patient transitions.
- Monitor patients in the outpatient clinic setting, with potential involvement in dialysis rounds or follow-up for dialysis patients.
- Order and interpret laboratory and diagnostic tests; adjust treatments based on results.
- Document patient encounters thoroughly in electronic health records (EHR).
- Participate in quality improvement initiatives and patient advocacy.
- Maintain up-to-date knowledge of nephrology best practices through continuing education.
Qualifications
- Master's degree in Nursing from an accredited Nurse Practitioner program.
- Current certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) through ANCC or AANP.
- Active Nurse Practitioner license in [State] (or eligibility for licensure).
- DEA licensure and prescriptive authority.
- Minimum of 1-2 years of experience as a Nurse Practitioner; experience in nephrology, internal medicine, or hospitalist settings preferred.
- Dialysis experience (hemodialysis or peritoneal dialysis) is a plus.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal abilities for patient education and team collaboration.
- Proficiency with EHR systems.
Preferred Skills
- Certification as a Certified Nephrology Nurse - Nurse Practitioner (CNN-NP) is a plus.
- Experience managing patients with ESRD or on dialysis.
- Bilingual skills (if applicable to your patient population).
