What are the responsibilities and job description for the Psychiatric Rehabilitation Program (PRP) Counselor(RC) position at NEO Health Care Services, Inc.?
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Meet with clients regularly and consistently to work on client goals (six times a month for adults, four times a month for minors).
- Ensure that the Progress notes, monthly summaries and Intake assesements, are done in a timely manner.
- Conduct regular in-home or community-based client visits to provide individualized case management, counseling, coaching, and skills training as needed.
- Link clients to government and community resources as needed and assist in clients obtaining those resources.
- Ensure client maintenance of physical and mental health and well-being.
- Provide and/or coordinate transportation to and from scheduled appointments.
- Complete all documentation including but not limited to, notes, monthly progress summaries, and quarterly IRP reviews, in a timely and accurate manner.
- Report regularly to immediate supervisor and engage in supervision and training.
- Ensure compliance with all agency policies and procedures and state regulations governing the provision of Psychiatric Rehabilitation Services.
QUALIFICATIONS:
BA degree in human services , art, or social services with at least one (1) year of experience working with clients with severe emotional or psychiatric problems. Or, a high school diploma with some coursework in social services or human services with at least three year experience in providing therapeutic or rehabilitations services to clients with severe emotional problems.
- Ability to critical thinking, analyze, evaluate and develop solutions for client’s rehabilitation needs.
- Excellent interpersonal skills with the ability to effectively build relationships with individuals of all ages, ethnicity, education, and work experience.
- Understands and maintains confidentiality and all regulations regarding HIPPA
