What are the responsibilities and job description for the Part Time Case Manager position at Nemaha County Training Center, Inc.?
We are looking for a Part Time Case Manager to advocate for individuals with intellectual and developmental disabilities, as well as coordinate and develop their support plan. Coordination of services includes community services (medical and specialized services) and financial assistance services. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the individuals health and human services needed.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and person-centered
- Handle case assignments, draft service plans, and review individual progress
- Help individuals achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help individuals make informed decisions by acting as their advocate regarding their services and treatment options
- Interact with individuals to keep track of their progress and to ensure satisfaction
- Record applicable information, complete accurately all necessary forms and produce statistical reports
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations
