Position Overview
The Community Liaison plays a key role in growing our services by building strong, lasting relationships in the community. This person will create opportunities to build a referral bridge between physicians, case managers, discharge planners, social workers, and other referral sources.
This role is perfect for someone who enjoys connecting with people, helping families find care, and making a real difference in the lives of seniors.
Key Responsibilities
- Build and maintain relationships with:
- Physicians and medical offices
- Hospital discharge planners
- Case managers and social workers
- Rehabilitation centers, senior communities, and local organizations
- Create consistent referral opportunities by staying visible and engaged in the community
- Educate referral partners on Neighborly Hands services, including home care and nursing support
- Follow up on referrals and ensure a smooth transition for new clients and families
- Attend networking events, health fairs, and community outreach activities
- Track referral sources and maintain strong communication with the internal team
- Represent Neighborly Hands with professionalism, warmth, and integrity at all times
Skills & Qualifications
- Experience in healthcare, home care, marketing, or community outreach preferred
- Strong communication and relationship-building skills
- Comfortable speaking with professionals and families in a caring and confident way
- Organized and able to manage multiple relationships at once
- Self-motivated and able to work independently
- Basic computer skills and ability to track outreach efforts
- Knowledge of the local healthcare system is a plus
