Key programs include the CaseManagement Program (CMP), Community Care Network Program (CCNP), VIP Elder Justiceand Crime Victims Program, NY CONNECTS, multiple senior centers, Caregiver SupportProgram, and SHOPP Supportive Options and Services through the SARA Project.Position Summary:The Social Worker Team Leader oversees one of three CMP case management teams, providingleadership, clinical supervision, and direct services to frail older adults with long-term care needswho wish to remain in the community. This role ensures high-quality, strengths-based casemanagement, supports program administration, and fosters a collaborative team environment.Key Responsibilities:Direct Services: Conduct and monitor client assessments, reassessments, and care plans Provide clinical interventions and maintain complete, accurate documentation Authorize services and manage waiting lists in consultation with the Program Director Coordinate with community resources and referral sources to ensure timely, appropriateservicesSupervision & Leadership: Lead and supervise a team of case managers and direct service workers Maintain a collaborative, team-based approach to caseload management Conduct individual and group supervision, performance evaluations, and staffdevelopment Manage schedules, timesheets, and support social work student field placementsProgram Administration: Monitor case management goals, outcomes, and service delivery quality Maintain program records and reports for funders Conduct outreach, community presentations, and represent SHOPP at professionalmeetings Support program planning, policy implementation, and compliance with contractualobligations Maintain and update resource directories and ensure staff training/certificationrequirements are met Assist with recruitment, onboarding, and professional development of staffQualifications: Masters Degree in Social Work (MSW) from an accredited school Experience in social services, preferably with older adults and community-based casemanagement Strong leadership, clinical, problem-solving, and team-building skills Bilingual Spanish/English preferred Computer literacy mandatory; familiarity with NYC AGING Peer Place systems helpfulWork Schedule & Conditions: Hybrid schedule; minimum of two days per week in the office Availability for evenings and weekends to support program events Travel to community events and fieldwork as needed Required attendance at in-person meetings, trainings, and program eventsBackground Check Notice:All offers of employment are contingent upon the successful completion of a background check.SHOPP is an equal-opportunity employer that fosters a diverse, inclusive, and respectful workplace.
