Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.The Certified Nursing Assistant works to support the mission and vision of Neighborhood Healthcare (NHC) by supporting the nursing team with daily nursing care and services. This role will perform routine technical tasks and health screenings with participants. In addition, this role will assist participants with their basic needs, activities of daily living, and recreational activities.Schedule: Monday-Friday, 8:00 am-4:30 pm(30-minute lunch)ResponsibilitiesPerforms and records routine technical tasks for participants, such as vital signs (temperature, blood pressure, pulse, and respiratory rate), collection of specimens, intake and output, and massesAssists with admissions, transfers, and discharges of participants by safely transporting, lifting, and positioning patientsReinforces selected patient teachings initiated by registered nursesCleans, organizes, and maintains participant, program, and unit areas, including equipmentConsiders special needs and behaviors of specific patient age groupsPrepares and performs recreational activities with participants, as neededPerforms and assists with home visits, including activities of daily living (ADLs), transfers, and companionshipAccompanies participants to medical appointments and pharmacy drop-offs/pick-upsProvides personal care for participants, such a toileting, bathing, eating, transferring, dressing, and groomingAssists management in carrying out the activity program, including serving meals/snacks and performing meal countsProvides assistants to patients and families, as neededTransports patients and equipment, as neededDocuments all care and services provided to participantsMonitors and reports any variances to normal participants activity to the nurse for further assessmentFollows infectious disease precautions to prevent the spread of organismsAttends and actively participates in the intake and assessment process, as neededShares accountability for overall patient health outcomes by working in coordination with care teamsAttends center staff meetings and in-service educational training programsQualificationsEducation/ExperienceHigh school diploma/GED requiredCalifornia Nurse Assistant certification is requiredBasic Life Support (BLS) through the American Heart Association certification required prior to hire.One year experience working with the frail elderly population is requiredAdditional Qualifications (Knowledge, Skills and Abilities)Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with working with elderly populationsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 50 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
