PHARMACIST

As a Pharmacist, you will oversee and enforce the polices, procedures, and day-to-day operations related to the in-house pharmacy and facilitate the 340B Program. Maintain patient confidentiality and ensures compliance with HIPAA policies and procedures.  

ESSENTIAL DUTIES & RESPONSIBILITIES 

  • Conducts and evaluates medication histories, assesses compliance, and suggests modifications for optimal drug product selection in order to achieve desired outcomes; instructs patients on correct use of prescribed medications.
  • Instructs patients in correct use of prescribed medications and identify any barriers to patient compliance with drug therapy.
  • Individualizes medication regimens using sound principles and understanding of drug absorption, distribution, metabolism, and elimination to establish and improve to optimum patient benefit.
  • Effectively verifies patient medication orders for appropriateness and accuracy. Provides medication reconciliation and evaluates medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information.
  • Prevents, detects, and resolves medication-related problems. Makes evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team in a PCMH Model.
  • Prevents contraindication and performs duplicate therapy checks, in assessing and formulating recommendations to providers ensuring quality and economical drug therapy is performed daily.
  • Provides information, education, and counseling to patients about medication-related care.
  • Works under collaborate drug therapy management agreements or standing orders, as applicable.
  • Provides concise, applicable, and timely responses about medication queries from providers, staff and patients.
  • Identifies any barriers to patient compliance with drug therapy.

  • Utilizes INSPECT to ensure appropriate use and dispensing of controlled substances.
  • Follows appropriate policies and procedures along with state and federal regulations regarding the handling of controlled substances
  • Manages tasks completely and efficiently, using EHR and pharmacy management system 
  • Provides immunizations to patients when appropriate.
  • Communicates with payers to resolve issues that may impede access to medication therapies.
  • Ordering pharmacy supplies and controlling the inventory on a daily basis.
  • Enforcing best practice protocols when storing prescription drugs and controlled substances.
  • Comply with all state and federal laws and regulations regarding the practice of pharmacy
  • Comply with all security measures and quality standards outlined by Neighborhood Health policies and procedures. 
  • Actively participates in the 340B drug pricing program including appropriate use of sliding scale for uninsured and underinsured patients.
  • Help facilitate inventory management in the pharmacy taking into consideration the 340B program
  • Assists the pharmacy manager and director in reviews 340B data and evaluates for needed changes and areas of improvement.
  • Tracks and reviews quality data relevant to pharmacy and evaluates for needed changes and areas of improvement.

-Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of drugs designated as hazardous waste. 

-Complies with all state and federal laws and regulations. 

-Actively participates in development and implementation of related performance/quality improvement and patient safety activities. Reports errors and adverse reactions promptly. 

  • Performs duties in accordance with NHC Standards of Conduct and Mission Statement.
  • Performs related work as required and other duties (similar physical requirements and OSHA risk level) as assigned.  

REQUIRED SKILLS / ABILITIES 

  • Clinical—demonstrates a working knowledge of medical theory and practice to evaluate and provide patient care; skill in taking medical history to assess medical condition and interpret findings; maintains knowledge of, and acts in accordance with, current NHC policies and procedures. 
  • Problem solving – identifies and resolves problems in a timely manner and gathers and analyzes information skillfully.
  • Interpersonal skills—maintains confidentiality, remains open to others’ ideas and exhibits willingness to try new things; maintains friendly and supportive relationships with coworkers; talks through problems to keep channels of communication open and maintain a high level of trust; realizes the importance of, and practices, good customer service; and able to communicate effectively and build collaborative, supportive relationships with people of varying cultures, socio-economic backgrounds, languages, and educational levels. 
  • Oral communication—speaks clearly and persuasively in positive or negative situations, demonstrates group presentation skills. 
  • Written communication—edits work for spelling and grammar, presents numerical data effectively and is able to read and interpret written information.
  • Planning/organizing—prioritizes and plans work activities, uses time efficiently. 
  • Quality Control – understands the importance of compliance standards and pays close attention to accuracy and detail when performing duties; demonstrates a working knowledge of how grant requirements and federal/state laws impact the day-to-day operations of NHCI’s Medical and Pharmacy Department. 
  • Adaptability – adapts to changes in the work environment, manages competing demands and is able to deal with frequent change, delays or unexpected events.
  • Dependability – consistently at work and on time, follows instructions, responds to management direction and solicits feedback to improve performance.
  • Safety and security – actively promotes and personally observes safety and security procedures, and uses equipment and materials properly.
  • Information Technology/Medical Equipment – proficient in the use of computers and other medical equipment/instruments to administer patient care and accurately retrieve/record patient data. 

 

REQUIRED QUALIFICATIONS: 

Education / Training 

Doctor of Pharmacy (PharmD) degree preferred or Bachelor’s degree in Pharmacy. 

Experience 

At least one (1) year of pharmacy experience preferred 

340B experience preferred.  

Licensure/Certification 

Immunization certification.  

Physical Requirements 

Prolonged standing and walking, some stooping, bending, and stretching. Requires eye-hand coordination sufficient to operate computer keyboard. Requires ability to lift and carry items weighing up to 50 lbs. Requires ability to handle and assist patients. Requires corrected vision and hearing to normal range to record, prepare, and communicate appropriate reports. Requires ability to work under stressful conditions. Requires exposure to communicable diseases and bodily fluids.  

Work Environment 

Normal pharmacy environment. Work may include irregular hours, including evenings. Involves frequent contact with other staff, physicians’ offices, patients, and the public. Contact may involve dealing with sick, angry, or upset people. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinic environment. OSHA Category I – Tasks involve contact with blood or body substances.
