Neighborhood Health is a dynamic, fully integrated Federally Qualified Health Center (FQHC) located in the heart of downtown Fort Wayne, Indiana. Neighborhood Health employs a workforce of 230 employees at multiple locations that offer a variety of services: medical, dental, optometry, chiropractic, behavioral health, WIC and pharmacy serving the Northeast Indiana region.
Neighborhood Health is looking for an Optometrist to join our team. The Optometrist will have the unique opportunity to ensure that the highest quality care is provided in a Patient Centered Medical Home environment.

As an Optometrist, you will:
- Manage the acute and long-term optical needs of NH patients by performing visual examinations using a full field of optometry instruments and techniques.
- Diagnose visual diseases, disorders, and deficiencies, and prescribe treatment according to scope of practice and NH guidelines.
- Ensure that patients receive appropriate education and referrals.
- Record physical findings and other pertinent information in patient’s electronic health record within the timeframe established by Quality Assurance program’s guidelines for timely documentation.
- Participate in Quality Assurance activities as assigned.
Requirements:
- Indiana Optometry license
Education Requirements:
- Graduate of an accredited school of Optometry
