Purpose

The HealthCare Coordinator II (HCC II) provides advanced operational and administrative support for School-Based Health Centers (SBHC) and ensures smooth, high-quality clinic operations across three (3) OR more assigned school sites. This position functions with a high degree of independence, offering enhanced operational expertise to eight (8) or more medical, dental, and mental health providers and healthcare educator(s). The HCC II serves as the primary point of contact for external stakeholders, including school district administrators, community organizations, and collaborating agencies. This role leads and participates in community outreach efforts and school- or district-sponsored events to increase student and family engagement with SBHC services. The HCC II may also provide remote coverage support for other SBHC sites as needed and contributes to the onboarding and training of new Health Care Coordinators, supporting consistent workflows and operational excellence across the program.  The HCC II upholds Neighborcare Health’s mission, goals, service commitments, and equity-focused care principles.

Compensation:    

  • The target wage range for this position is $26.62 per hour to $31.92 per hour.
  • Final offers are individually based on various factors, including skill set, years of experience, location, qualifications, work schedule and other job-related reasons.

Primary Responsibilities:   

Clinic Operations & Patient Support 

  • Provide a welcoming, professional presence to students, families, school staff, and community partners.
  • Manage patient check-in processes, demographic updates, registration forms, and insurance verification.
  • Support patient access by scheduling appointments, coordinating walk-ins, and facilitating follow-up care.
  • Ensure clinics are prepared for daily operations, maintaining clean and organized patient-facing areas.
  • Respond to in-person and telephone inquiries with excellent customer service.

Program Support and Quality Improvement

  • Participate in quality improvement initiatives and process evaluations.
  • Assist program leadership by identifying operational issues, recommending solutions, and supporting implementation.
  • Support orientation and training efforts for new clinical staff regarding SBHC workflows and procedures.
  • Assist with the onboarding and training of new Health Care Coordinators, ensuring consistency in clinic processes, service commitments, and administrative standards.

Administrative Coordination

  • Serve as an advanced operational resource for SBHC team members, supporting system navigation, workflow questions, and complex administrative needs.
  • Support the development and refinement of operational processes to improve efficiency and patient access.
  • Perform advanced administrative tasks including reporting, documentation, data entry, communications, and information management.

School and Community Engagement

  • Serve as the primary point of contact for school principals, administrators, district personnel, and community service organizations.
  • Maintain strong, collaborative relationships with school staff across three assigned sites.
  • Coordinate and participate in school events, district wellness initiatives, and community outreach activities that promote SBHC services.

Facilities and Logistics Support

  • Coordinate with facilities, IT, and other internal teams to address site-level maintenance, equipment, and technology needs.
  • Maintain adequate clinic supply levels and support logistics for multi-site operations.

Required qualifications for this position include:

  • Minimum 2 years of ambulatory clinic operations experience with scheduling experience within EMR/EDR
  • Bachelor's degree in nursing, public health, healthcare administration, public administration, business, or a related field.
  • Effective problem-solving with sound judgment and independence.
  • Highly organized with strong multitasking ability.
  • Comfortable working across multiple sites and adapting to changing needs.

Preferred qualifications for this position include:

  • Experience in community health or school-based health settings. 
  • Experience using EMR/EDR systems, especially for scheduling.
