Job Summary:
Nehalem Valley Care Center is seeking a compassionate and dedicated Resident Care Manager (RN or LPN) to assist our Director of Nursing in overseeing the daily operations of our Skilled Nursing clinical department. In this vital role, you will help ensure the highest standard of care for our residents while supporting compliance with state, federal, and local regulations, and upholding our center's policies and values.
Responsibilities:

Welcome and assist with new admissions to ensure a smooth transition

Collaborate with residents, families, and interdisciplinary care teams to address concerns and care needs

Monitor the 24-hour report for changes in resident condition; follow up with assessments, care plans, notifications, and consultations

Review and support psychoactive medication use, restraints, and restorative programs

Assist with the completion of the MDS (Minimum Data Set) process

Communicate resident condition changes to families and physicians

Conduct incident and accident investigations in collaboration with other clinical staff

Perform additional duties as assigned by leadership
Requirements:

Current RN or LPN license in the State of Oregon (or ability to obtain)

Strong clinical, communication, and organizational skills

Ability to work collaboratively with a multidisciplinary team
