Job Description:
Hours: 8:00 a.m. to 4:30 p.m., Monday-Friday
Wage: $26.86/hour ($23.86 base rate $3 differential for every hour worked)
Perform social work case management through assisting members in resolving personal issues involving finances, health, family and environment. This individual will educate members and families of Member Rights as well as rules and guidelines of the facility. Will collect data to assess education, vocation, social and potential discharge needs. Will draft and implement social work care plan interventions. Will participate in interdisciplinary teams and groups and will serve as an advocate for members.
Requirements / Qualifications:
Minimum Qualifications: Bachelor's degree in Social Work with certification in Social Work and one year experience in the field OR Bachelor's of Arts degree in a related field with certification in Social Work by grandfathering and two years of experience in a residential setting, public welfare, or community service work.
Knowledge of: Principals of social work, human behavior, community resources and services available for financial assistance, support groups for members/families, and discharge care and placement; and programs available through other State agencies.
Skill in: Interviews, assessments, interventions, care planning, basic problem solving, planning & organization.
Ability to: Work independently and as a team, communicate effectively written and orally, organize workload priorities to meet designated responsibilities and maintain the confidentiality of member information. Understanding of geriatric population, including persons with dementia, as well as long term care practices. Understanding of regulations governing social work activities.
