Job Title:Nurse Health Coach (RN, LPN, NP) – One-on-One Coaching
Location:Erwin, NC (hybrid and NC Virtual)
About the Role:
We are looking for a compassionate Nurse (RN, LPN, or NP)to provide one-on-one health coaching for our clients. You’ll help clients improve their wellness, manage chronic conditions, and achieve lifestyle goals through personalized coaching, education, and support while understanding how mental health affects your physical health.
Responsibilities:
- Conduct health and wellness assessments
- Develop individualized coaching plans
- Provide one-on-one sessions in-person or virtually
- Educate clients on nutrition, exercise, stress management, preventive care, etc.
- Track client progress and adjust plans as needed
- Maintain confidential client records
- Focus on natural or holistic health
- Do labs
- Do weekly podcasts
- Other duties as assigned
Qualifications:
- Licensed RN, LPN or NP in North Carolina
- Strong communication and motivational skills
- Experience in wellness, preventive care, or chronic disease management is a plus
- First Aid Mental Health Certification is required (practice will set this up).
- Health coaching/wellness certification is required (practice will set this up).
